Burcham Parents/Guardians: Fill out and submit this form to notify our school's attendance office of your child's absence(s). Use one form per child.

Per district policy, absences can only be verified if reported within 5 days. If absence is due to a doctor, dental or court appointment, a professional note is required in order to be verified as excused. Absences longer than 3 days require a doctor's note to be verified as excused. Attach the doctor's note to the google form.

If student is sick, please detail symptoms. If the symptoms are consistent with COLD/FLU/COVID, please call the office at (562) 420-2685 for the most updated guidance on returning to school.

Please allow the office up to 3 days to clear your child's absence.
